Program and Qualifications

The Choice Wireless LIFELINE service provides a FREE phone and 250 FREE local minutes of calling and 100 text messages every month to help families in need.

You can qualify for Lifeline in Nevada if you meet the following qualifications:


1. If you get assistance from one or more of the following programs:

  • Medicaid (not Medicare)
  • Food Stamps
  • Supplemental Security Income (SSI)
  • Federal Public Housing Assistance (Section 8)
  • Low Income Home Energy Assistance Program (LIHEAP)
  • National School Lunch (free program only)
  • Temporary Assistance for Needy Families (TANF)

OR

2. If your total gross annual household income is at or below 150% of the federally-established poverty levels set forth for the number of persons in your household.

If you qualify based on total household income, you must provide one or more of the documents listed below:

  • Prior year’s state, federal or tribal tax return
  • Retirement Pension benefit statement
  • Social Security benefits statement
  • Divorce decree or child support document
  • Veterans Administration benefits statement
  • Unemployment/Workers Compensation benefits statement
  • Federal or tribal notice letter of participation in Bureau of Indian Affairs General Assistance
  • Current year-to-date earnings statement from employer or three consecutive months’ worth of paycheck stubs from the previous 12 Months
